Patient Rights

 

As a patient, you have the following rights:

 

1. Right to Receive Respectful Care

 

â—‹ You have the right to receive care with dignity, respect, and without discrimination based on race, ethnicity, gender, sexual

   orientation, age, disability, religion, or any other characteristic.

 

2. Right to Be Informed

 

â—‹ You have the right to receive clear and understandable information about your health, diagnosis, treatment options, and

   prognosis. This includes the right to ask questions and receive explanations about proposed treatments.

 

3. Right to Privacy and Confidentiality

 

â—‹ You have the right to privacy regarding your medical condition, personal information, and any records related to your care.     All information about you will be kept confidential in accordance with privacy laws (e.g., HIPAA in the U.S.).

4. Right to Participate in Your Care

 

â—‹ You have the right to actively participate in decisions regarding your treatment. This includes the right to agree to or refuse

   any treatment or procedure, except in emergencies where consent may be implied.

 

5. Right to Choose Healthcare Providers

 

â—‹ You have the right to choose your healthcare provider(s) and be informed about the qualifications of the providers offering

   your care.

 

6. Right to Access Medical Records

â—‹ You have the right to access your medical records in accordance with applicable laws. You may request copies of your

   records, and we will make reasonable efforts to provide them to you.

 

7. Right to Receive Safe and Timely Care

 

â—‹ You have the right to receive care in a timely manner and in a safe environment that meets established standards of

   practice.

 

8. Right to Pain Management

 

â—‹ You have the right to have your pain assessed and addressed, including discussing options for pain management with your

   healthcare providers.

 

9. Right to Express Concerns

 

â—‹ You have the right to voice concerns, complaints, or grievances regarding your care. We encourage open communication

   and will make efforts to resolve any issues promptly.

 

10. Right to Know About Costs and Billing

 

â—‹ You have the right to receive clear information about the cost of your care and any out-of-pocket expenses, including

   details about insurance billing, co-pays, and financial assistance programs.

Patient Responsibilities

 

As a patient, you have the following responsibilities:

 

1. Provide Accurate Information

 

â—‹ You are responsible for providing complete, accurate, and up-to-date information about your health history, medications,

   allergies, and other relevant health information. This helps ensure that your healthcare providers can make informed

   decisions about your care.

 

2. Follow Medical Advice

 

â—‹ You are responsible for following the treatment plan and instructions provided by your healthcare providers, including

   medications, therapy, lifestyle changes, and follow-up appointments.

 

3. Respect the Rights of Others

 

â—‹ You are responsible for respecting the rights of other patients and healthcare providers. This includes maintaining a   

   courteous and respectful demeanor toward staff, other patients, and visitors.

 

4. Comply with Facility Policies

 

â—‹ You are responsible for following the rules and policies of the medical center, including safety protocols, infection control 

   measures, and visitation policies.

 

5. Ask Questions

 

â—‹ If you do not understand your diagnosis, treatment options, or care plan, you are responsible for asking questions. 

   Communication is key to ensuring that you make informed decisions about your health.

 

6. Notify Your Providers of Changes in Health

 

â—‹ You are responsible for informing your healthcare provider of any changes in your health condition, including new 

   symptoms, changes in medications, or changes in your personal circumstances that might affect your treatment.

7. Keep Appointments or Notify Changes

 

â—‹ You are responsible for keeping your scheduled appointments. If you are unable to attend, you should notify the medical       center as soon as possible to reschedule.

 

8. Follow Financial and Billing Policies

 

â—‹ You are responsible for understanding and fulfilling your financial obligations, including insurance coverage, copayments,       and any out-of-pocket expenses. Please notify us if you need assistance with billing or understanding your financial   

   responsibilities.

 

9. Maintain a Safe Environment

 

â—‹ You are responsible for following safety protocols, including infection control practices, and notifying staff if you notice any     safety hazards or concerns.

10. Respect Confidentiality

          â—‹ You are responsible for maintaining the confidentiality of your own medical information and respecting the privacy of other               patients, staff, and healthcare providers.
